CASE MANAGEMENT

CASE MANAGEMENT (also referred to as CARE MANAGEMENT) is the practice of having a single expert, often a social worker or a nurse, work with the individual, family and other professionals to plan and coordinate all of the health and social services needed to help the individual remain at home.
